ABSTRACT

【Objective】 To investigate the serological and molecular biological characteristics of para-Bombay phenotype. 【Methods】 ABO blood type, H antigen and Lewis blood type in one blood sample with discrepancy between forward and reverse blood typing were detected by serological method. Antibody screening and identification and cross-match test were also performed by serological method. ABO blood group genes were detected by PCR-SSP, and FUT1 and FUT2 genes were sequenced. 【Results】 The serological test showed that the Para-Bombay phenotype was Ah secretion. The ABO blood group gene was AO2. FUT1 sequencing revealed two mutations: 235G>C and 881_882delTT. While FUT2 sequencing showed only one mutation 357C>T. 【Conclusion】 The discovery and accurate identification of blood groups is necessary to ensure the safety of blood transfusion. Blood donors of rare blood groups should be informed and recruited to the team of rare blood donors.

ABSTRACT

Osteoporosis seriously threatens the living quality of people, especially the elderly, and causes a huge economic burden to society. In the past, bisphosphonates, denosumab and other first-line drugs were used in the treatment of osteoporosis. However, these drugs can only inhibit bone resorption, but can not promote bone formation. Studies have shown that mesenchymal stem cells (MSCs) can be used to treat osteoporosis, however, it has some defects and deficiencies, such as genetic instability, limited cell survival and increased risk of cancer. However, mesenchymal stem cell-derived exosomes (MSCs-Exos) can regulate the differentiation and proliferation of osteoblasts by mediating wingless and int-1 (Wnt)/β-catenin and mitogen-activated protein kinase (MAPK) signaling pathways, promote bone regeneration, and thus has an impact on osteoporosis. In this paper, preclinical studies on MSCs and MSCs-Exos in the treatment of osteoporosis in recent years were reviewed, in order to provide a new idea for the treatment of osteoporosis.

ABSTRACT

Objective:To investigate the effect of passive motion and immobilization on shoulder function early after arthroscopic repair of rotator cuff tears.Methods:A retrospective case-control study was conducted to analyze the clinical data of 78 patients with rotator cuff tear admitted to Xinhua Hospital, Shanghai Jiaotong University School of Medicine from January 2016 to December 2017. There were 36 males and 42 females, aged 35-78 years [(62.7±3.2)years]. There were 36 patients with medium-sized tears (1-3 cm), 31 with small tears (<1 cm), and 11 with partial articular supraspinatus tendon avulsion (PASTA). All patients underwent arthroscopic rotator cuff repair. Forty-three patients started rehabilitation exercise immediately after operation (motion group). Thirty-five patients were immobilized with shoulder abduction brace for 6 weeks, and started rehabilitation exercise at week 7 (immobilization group). The range of motion, visual analogue pain score (VAS), simplified shoulder joint function test (SST) and Constant shoulder joint score were compared between the two groups before surgery, 6 weeks, 3 months and 12 months after surgery. The healing results were assessed by ultrasound 12 months after surgery. Complications were observed.Results:All the patients were followed up for 12-16 months [(13.7±1.3)months]. There were 7 patients with shoulder joint stiffness in each group (motion group: 16%, immobilization group: 20%) ( P<0.05). There were no significant differences between the two groups in VAS, SST or Constant score at postoperative 6 weeks, 3 months and 12 months ( P>0.05). The forward flexion and external rotation with the arm at the side in immobilization group was (124.9±12.9)° and 25(20, 30)° at postoperative 6 weeks, significantly improved in motion group [(136.6±16.7)°, 30(25, 40)°] ( P<0.05). There were no significant differences between the two groups in forward flexion and external rotation with the arm at the side at postoperative 3 and 12 months ( P>0.05). There were no significant differences between the two groups in internal rotation at postoperative 6 weeks, 3 months, and 12 months ( P>0.05). All rotator cuffs were healed verified by ultrasound at postoperative 12 months. No infection or implant displacement occurred after operation. Conclusions:For arthroscopic repair of medium-sized tears, small tears and PASTA, early postoperative rehabilitation exercises have advantage in improving range of motion only at early stage when compared to immobilization, which disappears with time. Moreover, the two methods have no significant differences in improving postoperative pain and shoulder function.

ABSTRACT

Objective:To identify potential risk factors of varus collapse after unstable proximal humerus fracture treated with locking plates.Methods:A retrospective case series study was conducted on data of 146 patients with unstable proximal humerus fracture stabilized by locking plates at Xinhua Hospital, Shanghai Jiaotong University School of Medicine from January 2008 through December 2014. These patients were classified into varus collapse group ( n=39) and non-varus collapse group ( n=107) according to the occurrence of varus collapse. The gender, age, bone mineral density, cause of injury, fracture Neer classification, fracture type (varus or valgus), surgical timing, surgical techniques (medial support, cancellous bone graft, suture augmentation), number of humeral head screws and reduction quality were recorded. Potential risk factors were evaluated using univariate analysis and multivariate Logistic regression. The subjective reliability analysis was performed for Neer classification and medial fracture assessments. Results:Varus collapse group had higher ratio of osteoporosis, varus fracture, lack of medial column support, absence of suture augmentation and varus malreductin compared to non-varus collapse group ( P<0.05). While the two groups had no significant differences in gender, age, fracture classification, allogeneic cancellous bone transplantation and number of humeral head screws ( P>0.05). Moreover, the Logistic regression analysis indicated that osteoporosis, varus fracture, lack of medial column support, absence of suture augmentation and varus malreduction were major independent risk factors for varus collapse in proximal humerus fractures ( P<0.05). Among these risk variables, the lack of medial column support showed the strongest correlation of varus collapse after proximal humerus fractures treated with locking plates ( OR=9.62), and varus malreduction was another remarkable risk factor ( OR=8.39). The reliability of Neer classification and medial fracture assessments between interobservers and intraobservers was good. Conclusion:The risk factors for varus collapse after unstable proximal humerus fracture treated with locking plate are osteoporosis, varus fracture, lack of medial column support, absence of suture augmentation and varus malreduction.

ABSTRACT

Objective To observe the clinical effect of tigecycline combined with cefoperazone sulbactam for the treatment of pneumonia caused by muitidrug-resistant acinetobacter baumannii pneumonia. Methods 68 patients with muitidrug-resistant acinetobacter baumannii pneumonia were randomly divided into two groups, control group and observation group, with 34 patients in each group. The control group was treated with cefoperazone sulbactam, and the observation group was treated with tigecycline combined with cefoperazone sulbactam. The level of PCT, CRP, WBC and clinical cure rates, as well as microbiological eradication and side effect, before and after treatment were collected and compared. Results After treatment, the level of PCT, WBC and CRP in two groups was decreased, and the observation group was more significant than the control group(P 0.05), while the clinical cure rates and microbiological eradication in observation group were significantly higher than the control group (P<0.05). Conclusion With a higher rate of clinical efficacy and a lower rate of side effect, the combination treatment with tigecycline and cefoperazone sulbactam would be a promising alternative for the treatment of muitidrug-resistant acinetobacter baumannii pneumonia infection.

ABSTRACT

Objective To evaluate the treatment effect of Lobaplatin combined with Aidi Injection on patients with advanced NSCLC, and observe the influence on the level of serum VEGF by the treatment. Methods 60 patients, with advanced NSCLC and malignant pleural effusion, were divided into two groups (observation group and control group) randomly, with 30 cases in each group. All patients were treated with thoracentesis and inserted central venous catheter for the drainage of pleural effusion. After exhaustion of pleural effusion, the control group was treated with Lobaplatin by pleural perfusion, and the observation group was treated with Aidi Injection in addition to the treatment on control group. The treatment, side effects and quality of life and the level of serum VEGF were compared. Results After treatment, it is significant that clinical effects, side effect and KPS score of living quality of the observation group was better than that of the control group (P<0.05). The level of serum VEGF was significant lower than that before treatment in both groups (P<0.05). The decrease of VEGF serum level in the observation group was significantly lower than the decrease in the control group (P <0.05). Conclusion Lobaplatin combined with Aidi Injection was one of the effective method for the treatment of NSCLS with malignant pleural effusion, and was worthy of clinical application.

ABSTRACT

We produced high pathogenic avian influenza H5N1 haemagglutinin protein HA1 in recombinant Pichia pastoris in a 10 L fermentor, to establish a high-density cell fermentation method. We studied the effects of different factors such as culture temperature, induced temperature, methanol feeding methods, trace elements on the growth of Pichia pastoris, the yield and the biologic activity of recombinant HA1 protein. The culture temperature in pre-induced and induced stage were optimized at 25 degrees C to adapt cell growth and recombinant protein expression, and induced temperature at 25 degrees C also resulted in higher biologic activity of rHA1 than at 30 degrees C. The binding activity of rHA1 against a wide-spectrum neutralizing antibody was susceptible to the presence of any trace elements, although trace elements would essentially benefit for the cell fermentation. As a conclusion, the expression level of rHA1 produced with optimized fermentation process reached 120 mg/L, which was 10.5 times higher than the one produced in regular shaking flask. The resultant high-density cell fermentation can likely produce rHA1 of H5N1 in large scale.
